Community Health Quality Recognition
The Community Health Quality Recognition (CHQR) Dashboard provides visualization, as well as national and state-level summaries, of the CHQR badges awarded each year. CHQR badges recognize Health Center Program awardees and look-alikes (LALs) that have made notable achievements in the areas of access, quality, health equity, health information technology, and social risk factors screening using Uniform Data System (UDS) data from the most recent reporting period. HRSA first awarded CHQR badges in 2021 for the 2020 UDS reporting period.

NCQA has helped to build consensus around important health care quality issues by working with large employers, policymakers, doctors, patients and health plans to decide what’s important, how to measure it, and how to promote improvement.
